Acute myocardial infarction is one of the most dangerous cardiovascular disorders. The main objectives of the treatment are to prevent recurrent cardiac events. A careful assessment of cardiovascular risk factors in young patients is, therefore, of utmost importance. The factors influencing clinical course in young patients are also discussed, as compared to the rest of the population. Retrospective study of 36 patients (23 men) aged less than 45 years, admitted for an acute myocardial infarction between 1980 and 1999 at the cardiology department of the University Hospital of Lausanne, Switzerland. The female to male ratio was 1.1. There were no significant differences in the risk factors of the patients, except for cigarette smoking in women. Seventy two percent of the patients were admitted in the month of the infarction. The anterior myocardial infarction was found in 33% of the patients, and the majority of the cases (75%) had a Thrombolysis In Myocardial Infarction (TIMI) flow grade 3. The mean ejection fraction was 53%, and there was one in-hospital death. Thirteen patients (36%) had a second infarction during the follow-up period, including four deaths. The cumulative incidence of the first ischemic event was 80% at 5 years. Several risk factors (notably diabetes and tobacco smoking) were significantly more frequent in the young patients.
